If your child has a medical condition, asthma, allergy or requires medication/medical equipment to be kept in school, you will need to complete a Pupil Medical Information Form.  (If your child needs a special diet as a result of any allergies or medical condition, you will also need to notify our caterers, Edwards and Ward - see Lunch and Snacks.)

It is the responsibility of the parent/carer to advise the school of any changes to their child's medical information, at any time during the school year.

ALL medicine brought into school should be handed into the school office and a medicine instruction form should be completed.  

The school office can advise you of the correct form to be used, and provide you with a printed copy, but if you are leaving medicine with Morning Club you may prefer to print off and complete a copy ahead of time.
